Understanding the Psychedelic-Assisted Therapy Landscape

This is the most legally complex modality in holistic health. ICONIC Board's Phase 1 pathway is scoped specifically to legally clear practice — Ketamine-Assisted Therapy and Psychedelic Integration coaching — and will expand as the federal regulatory landscape matures. The following context is essential for practitioners considering this pathway.

Ketamine — Phase 1 (Active)

FDA-approved (Schedule III controlled substance) for treatment-resistant depression since 1970. Esketamine nasal spray (Spravato) approved 2019. Off-label IV infusion widely practiced. Legally practiced in all 50 states by licensed clinicians. Estimated 1.6M+ patients annually.

Psilocybin — Phase 2 (Forthcoming)

Schedule I federally; Oregon (Measure 109, 2022) and Colorado (Prop 122, 2022) have established state-level facilitated service frameworks. Additional state legalization efforts active in 10+ states. IBC pathway forthcoming upon further regulatory clarity.

MDMA & Others — Future Phase

FDA advisory committee reviewed MDMA-AT (2024); regulatory pathway developing. DMT, ibogaine, and mescaline remain Schedule I federally. IBC will establish pathways for each modality individually as legal frameworks mature. Not included in current pathway.

ICONIC Board of Holistic Health — Formal Recognition

ICONIC Board of Holistic Health recognizes the following Ketamine-Assisted Therapy (KAT) and Psychedelic Integration training programs as meeting the prerequisite education requirements for the corresponding IBC holistic health credential tiers. Graduates of the programs listed below may be eligible to apply for the relevant IBC credential — subject to meeting all additional Board requirements, including applicable licensed clinical practice prerequisites at Tiers II and above.

ICONIC Board credentials the professional practice of psychedelic-assisted therapy practitioners — the therapeutic relationship, ethical framework, informed consent process, harm reduction principles, and integrative client-centered philosophy that define responsible practice. IBC credentialing is independent of licensure and does not authorize the administration of controlled substances. Practitioners must hold all required state clinical licenses for the specific services they provide.

Important: Recognition of education pathways does not constitute automatic credentialing. All applicants must independently satisfy ICONIC Board's requirements for documented client practice hours, ethics attestation, CE compliance, and full application review. For Tier II (IBC-HHP™) and above, applicants in clinical KAT roles must hold a current state clinical license (LPC, LCSW, LMFT, RN, NP, PA, MD, DO, or ND). Integration coaches applying at Tier I (IBC-HHA™) are exempt from the licensed clinical prerequisite.

Licensed Clinical Prerequisite — Tiers II–IV

Clinical KAT Practitioners: Licensed Clinical Practice Required at Tier II and Above

Ketamine-Assisted Therapy involves the supervised administration or monitoring of a Schedule III controlled substance — a clinical act requiring licensure in all 50 states. ICONIC Board requires that applicants for IBC-HHP™ (Tier II) and above who practice in clinical KAT roles hold a current, active state clinical license — LPC, LCSW, LMFT, RN, NP, PA, MD, DO, or ND. Psychedelic Integration coaches who do not administer or supervise substance sessions may apply for IBC-HHA™ (Tier I) without a licensed clinical prerequisite.

Tier I — IBC-HHA™
Psychedelic Integration Coaches. No licensed clinical prerequisite required. Education + documented practice hours.
Tier II — IBC-HHP™
Clinical KAT practitioners. Active state clinical license required (LPC, LCSW, RN, NP, MD, DO, ND, or equivalent).
Tier III — IBC-HHE™
KAT educators and supervisors. Active clinical license + documented teaching/supervisory role in recognized KAT training program.
Tier IV — IBC-HHD™
Researchers, thought leaders, doctoral-level contributors. Active clinical license + doctoral education or equivalent research/institutional leadership.

  • Certificate programs from IBC-recognized training institutions covering psychedelic harm reduction, integration frameworks, and client-centered support practice (minimum 40 contact hours)

ICONIC Board Certified Holistic Health Associate
Graduates of recognized integration coaching programs may be eligible to apply for the Associate credential. No licensed clinical prerequisite required at this tier. Documented client support hours and ethics attestation are required.
Recognized Clinical Programs — Ketamine-Assisted Therapy (Licensed Clinicians)
Ketamine-Assisted Therapy Practitioner Training
  • Polaris Insight Center KAP Training — Comprehensive Ketamine-Assisted Psychotherapy certification (licensed clinicians; 50+ hours)
  • KRIYA Institute Ketamine Training — KAP professional training for licensed mental health clinicians
  • Fluence KAP Training — Ketamine-Assisted Psychotherapy (licensed mental health professional track)
  • MAPS Therapist Training — Full MAPS-certified psychedelic-assisted therapy training (clinical phase, currently KAP-applicable)
  • Integrative Psychiatry Institute (IPI) — Psychedelic-Assisted Therapy certificate for licensed clinicians
  • Licensed clinicians (LPC, LCSW, LMFT, RN, NP, MD, DO, ND) who have completed comprehensive KAT training with minimum 100 documented supervised clinical contact hours in ketamine-assisted sessions
Tier II — Practitioner
IBC-HHP™
ICONIC Board Certified Holistic Health Practitioner
Graduates of recognized clinical KAT programs holding a current state clinical license may be eligible to apply for the Practitioner credential. Documented supervised clinical hours are required in addition to education prerequisites.
Recognized Advanced & Teaching Programs — Clinical Supervisors & KAT Faculty
Psychedelic-Assisted Therapy Educator & Supervisor Level
  • MAPS Senior Therapist / Supervisor designation — recognized training supervisors within MAPS-certified therapy programs
  • Fluence Faculty / Trainer designation — lead trainers and clinical supervisors in Fluence KAP programs
  • CIIS Certificate in Psychedelic-Assisted Therapies and Research — California Institute of Integral Studies (graduate-level; 100+ hours) held by licensed clinicians with documented supervisory roles
  • Licensed clinicians with active IBC-HHP™ credential + minimum 500 documented clinical KAT hours + documented supervisory or instructional role in a recognized KAT training program
  • Published educators in psychedelic-assisted therapy with documented faculty appointments at recognized training institutions
Tier III — Educator
IBC-HHE™
ICONIC Board Certified Holistic Health Educator
Applicants meeting the above criteria may be eligible to apply for the Educator credential. Active clinical license and documented supervisory or teaching practice are required at this tier.
Recognized Doctoral-Level or Equivalent Research & Leadership
Doctorate-Level Psychedelic-Assisted Therapy Credentials
  • PhD, PsyD, or MD/DO in psychology, psychiatry, or closely related field with documented psychedelic research or clinical leadership
  • MAPS Principal Investigator or Senior Research Scientist designation with verifiable peer-reviewed publications in psychedelic-assisted therapy research
  • Faculty at degree-granting institutions in programs with documented psychedelic research or curriculum development (e.g., CIIS, Naropa University, Yale psychedelic research programs)
  • Senior program directors or founders of nationally recognized KAT training institutions with 15+ years documented clinical and leadership practice
  • Combined advanced clinical credentials + 2,000+ documented KAT contact hours + peer-reviewed research or significant institutional contribution to the field
Tier IV — Doctorate
IBC-HHD™
ICONIC Board Certified Holistic Health Doctorate
Applicants meeting the above criteria may be eligible to apply for the Doctorate credential. Doctoral documentation, active clinical license, and full Board assessment are required at this tier.
Advanced Pathway Requirements — Doctoral + Examination
Diplomate Pathway Requirements
  • Hold a current, active IBC-HHD™ (Doctorate credential) in good ethical standing
  • Documented post-IBC-HHD™ practice with 2,500+ cumulative professional contact hours in psychedelic-assisted therapy or integration
  • Pass the ICONIC Board Diplomate Qualifying Examination — a structured written examination assessing advanced clinical competency, ethics, and field leadership
  • Two letters of professional recommendation from IBC-credentialed practitioners at IBC-HHD™ or above
  • Sustained continuing education compliance throughout the credentialing period
The Diplomate designation is the first of two advanced tiers awarded through structured examination — representing demonstrated mastery at the highest clinically-practiced level in holistic health.
Tier V — Diplomate
IBC-DHH™
ICONIC Board Diplomate in Holistic Health
Candidates meeting the examination and practice requirements may be eligible for the Diplomate designation. Extended application review and Board examination are required. Exam fee: $795.
Board-Conferred Distinction — Nomination Required
Fellow Pathway Requirements
  • Hold a current, active IBC-HHD™ (minimum) or IBC-DHH™ in good standing
  • Nomination by two sitting ICONIC Board Fellows or Board Directors
  • Distinguished contribution to the psychedelic-assisted therapy field: published research, curriculum development, institutional leadership, or national/international policy influence
  • 15+ years of sustained, documented professional practice and contribution to the profession
  • Board review and affirmative vote by the ICONIC Board Fellowship Committee
The Fellow designation is the highest distinction ICONIC Board confers — board-selected and reserved for practitioners whose careers have shaped the profession at scale. Fellowship is not applied for; it is awarded.
Tier VI — Fellow
IBC-HHF™
Fellow of the ICONIC Board of Holistic Health
Fellowship is board-conferred and not directly applied for. Eligible candidates are nominated by sitting Fellows or Board Directors and reviewed at the Board's discretion.

MDMA-AT regulatory review ongoing. DMT, ibogaine, and mescaline pathways will be evaluated on their own timelines. Indigenous and ceremonial traditions (ayahuasca, peyote, iboga) will be addressed with dedicated cultural sensitivity frameworks.
